Purpose of this Consent Form

This form seeks parental/guardian consent for the student named below to participate in the Prom Court selection process and activities associated with this honor. By completing this form and signing below, you grant permission for your child to:

1. Run for and potentially be elected to the Prom Court.

2. Participate in all related Prom Court events, ceremonies, and photo sessions.

3. Adhere to the selection process, voting procedures, and any applicable contest guidelines.

Before Signing please ensure you have read the full list of requirements, policies, and voting practices for the 2025 Social Season Soirée Prom Court. This can be found under the Events Tab and is a required reading during the checkout portion of your ticket purchase.

Expectations & Requirements

1. Eligibility:

- The student must meet any required criteria set by the event organizers (e.g., grade level, homeschool requirements, GPA, community involvement, etc.).

2. Behavior & Conduct:

- The student is expected to abide by the event’s Code of Conduct and all policies.

- Failure to comply may result in disqualification from the Prom Court.

3. Dress Code:

- The student must comply with the formal dress code guidelines provided by the organizers.

4. Voting & Results:

- Voting may involve peers, friends, or family. The results are final once announced.

5. Media Release (Optional):

- By participating, the student may be photographed or recorded for promotional or celebratory purposes.

Liability & Waiver

-I understand that event organizers, volunteers, and venue staff are not responsible for any personal belongings, injuries, or unforeseen incidents that may occur at or in transit to/from the event.

- I acknowledge that, should my child be elected to the Prom Court, they will be publicly recognized at the event.
